Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a hazard in that the employer did not institute a policy and procedures for conducting periodic and regular inspections of horizontal injection molding machines to insure that safeguards were in operating condition and in proper adjustment in accordance with American National Standards Institute standard ANSI B151.1-1990: (a)11/17/94, Plastics Department - Horizontal injection molding machines including 14 York, 3 Imco, 1 Fellows, and 13 Boy machines were not subjected to periodic and regular inspections. One feasible means of abatement is: At the start of each eight hour shift and after each die change inspect all safety features of the machines including but not limited to limit switches; electric, pneumatic and hydraulic systems; heat mechanisms; and mechanical safety features. Do not operate the machine unless all safety features are working properly.
