Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to hazard of being struck by rolls of materials:
(a)In Unwind Department  Employees were exposed to hazard of being struck
by rolls of
material that are positioned between the turret arms of the unwind
machine and held in
this position using an altered and inoperable latch lock assembly which
is connected to
the pneumatic chuck assembly.  Rolls of material may break free due to
inoperable latch
lock assembly.
Among other methods one feasible and acceptable abatement method to
correct this
hazard is to ensure that proper inspection and maintenance procedures are
followed to
ensure the spindle latching and latch lock assembly do not become worn to
the
point
where they will be unsafe.  Also, the manufacturer should be contacted
for their
assistance before any modifications are made to mechanical control
functions of the
unwind machine.
