Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to serious fall hazards from the mezzanine and second floor levels and serious crushing injuries on the mezzanine and first floor levels. A feasible and useful method to correct the hazard, among other methods, is to provide electrically and mechanically interlocked gates to prevent them from being opened until the platform has reached that level: Loading Dock: The Langley vertical reciprocating conveyor was not equipped with electrically/mechanically interlocked gates exposing employees to serious fall and crushing hazards.
