recognized hazards that were causing or likely to cause death or serious

The hazard of entering and working in a 20 inch diameter gas main where
there
was the potential for a toxic and oxygen deficient atmosphere.
Among other methods one feasible and acceptable abatement method to
correct
this hazard:  Institute a confine space entry program meeting the
stipulations
of the NIOSH criteria document No. 80-106, December 1979, which would
provide
for continuous air monitoring for Oxygen, Methane, Co, and any chemicals
used
which may create a hazardous atmosphere in the approximately 700 gas main,
where three or more employees worked.  Ensure that rescue personnel in
addition to the attendent are always outside of the entry point or can be
contacted immediately to implement rescue
