Section 5(a)(1) of the Occupational Safety & Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees,
in that, employees were
exposed to the hazard of delayed rescue and possible injury during entry
and exit from
manhole:
CP402/427; Manhole3A, Deer Island:
On November 8, 1993, and December 2, 1993, employees were exposed to the
hazard
of delayed rescue and possible injury during entry and exit from
Manhole3A, on West
Perimeter Road.
A feasible and useful method to correct this hazard, among other methods,
is to institute an
effective confined space entry program meeting the requirements of OSHA
29 CFR 1910.146
and/or ANSI Z117.11989.
The program should include:
1)Effective training of the authorized entrants, attendants and all
affected employees.
2)Develop, implement, and correctly complete all portions of entry
permit.  The permit
should specify the nature of the work to be conducted and all necessary
equipment,
including rescue equipment.
3)Use of effective retrieval device such as a winch and tripod with a
fall arrest system
where self rescue may fail and vertical retrieval is necessary.
4)Continuous air monitoring where there is a potential for entry of toxic
or hazardous air
contaminants.
