Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to the hazards of crushing injuries, having part of their body caught between the moving elevator and the hoist way. A feasible and useful method to correct this hazard, among other methods, is to properly adjust the electrical interlock switch: Jobsite: The interlocks for the elevator car gate were not functioning properly in that the elevator would continue to operate while the car gate was partially open, exposing employees to serious crushing injuries.

Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to the hazards of crushing, in that the limit switch on the hoist was not in place so that the hoist could two-block or overtravel, exposing employees to serious crushing injuries. A feasible and useful method to correct this hazard, among other methods, is to install an upper limit switch: Jobsite: The hoist was not equipped with a upper limit switch.
