Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to the hazards of having part of their body caught between the moving elevator and the hoistway. A feasible and useful method to correct this hazard, among other methods, is the installation of car doors or gates that would prevent employees from having any part of their body outside the periphery of the elevator car unless the elevator car is within the landing zone and that were electrically interlocked to prevent the elevator car from moving unless the elevator car doors or gates are in the closed position: 285 Medford Street, Charlestown, MA: The interlocks for the elevator car gate were not functioning properly in that the elevator would continue to operate while the car gate was partially open, exposing employees to serious crushing injuries.

Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to a potential crushing or being struck by the load in the event of a brake failure: A feasible and useful method to correct this hazard, among other methods, is to clean or replace the brake linings: Jobsite: The hoist drum had visible signs of petroleum products coming in direct contact with the brakes of both trigger hoists.

Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to potential crushing hazards in the event of a wire hoist rope failure. A feasible and useful method to correct this hazard, among other methods, is to open the entry area for the hoist line coming through the roof so the rope would spool properly: Jobsite: The opening in the silo roofs restricted the wire rope from spooling properly on the hoist drum.
