that were causing or likely to cause death or serious physical harm to

employees were exposed to the hazard(s) of:
a)Employees placing propane tanks in blast cabinets were exposed to
struck-by/crushed by
hazards created by improper installation of wire rope clips (also known as
saddle clamps, U-
Bolts or Crosby Clips) at the terminations where connected to heavy metal
doors (weighing
approximately 1,000 lbs each).  One saddle clamp was installed with saddle
on the dead ends
of the wire rope instead of a minimun of 3 clamps, at each termination,
with saddles on the live
end of the rope, on or about 02/12/97.
Feasible and acceptable abatement methods to correct this hazard include:
a) Install a minimum of three Crosby clips, with saddles on the live ends
of
wire ropes, for
each wire rope connection.
b) Install a minimum of three Crosby "Fist Grip" style clips at each wire
rope connection.
