Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrical shock and scalding burns from hot water: FACILITY: The employer did not establish and implement hazardous energy control procedures for employees during servicing and maintenance of equipment, such as, (but not limited to) the heating boilers. 1. The employee replaced two (2) safety relief valves on the steam boiler #3. 2. The employee was performing a manual "Blow Down" on the steam boiler #3. Among other methods, one feasible and acceptable method of abatement is to: 1. Follow the American National Standard Institute ANSI Z244.1-2003 Standard Control of Hazardous Energy Lockout/Tagout and Alternative Methods. 2. Establish and implement energy control procedures that clearly and specifically outline equipment and machine specific step-by-step energy control procedures for shutting down, isolating, blocking, securing, and testing machines or equipment to control potentially hazardous energy. 3. Require and enforce lockout/tagout energy control procedures before any maintenance or servicing of machines or equipment. 4. Provide training to employees allowed to perform the maintenance and servicing work as well as to employees who may be affected by the work.
