Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing hazards due to the Tar Kettle (Lugger) not being
properly rigged for
hoisting.
101 Broad Street, Stamford, CT:  The 55 gallon Tar kettle (lugger) was not
properly rigged for
hoisting, in that it was equipped with three hoisting attachment points,
but was hoisted with a
3/4 inch diameter frayed fiber rope attached to the front handle, and was
attempted to be
lowered from a roof top 18 feet above ground.
Among other methods, one feasible and acceptable abatement method is to:
1.  Use rigging equipment and devices specifically constructed and
approved for such purposes.
2.  Conduct daily inspections of all rigging equipment, and remove any
equipment found to be
defective.
3.  Follow all manufacturers specifications and guidelines.
4.  Train all employees in safe rigging and hoisting practices.
