General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees, in that employees were
exposed to flying projectiles of hard
rubber tires and metal rims while beading and seating rubber tires to the
metal rims:
Shop:  The employer did not ensure through not only providing proper and
appropriate tools and equipment,
but also direct observation, instruction and training that the employees
engaged in the tasks, such as (but not
limited to) beading and seating rubber tires onto metal tire rims
utilizing pneumatic tools were performing the
tasks safely and appropriately at all times.
On or about November 4, 2011, a Power King 4.80-8 nylon 6 P.R. 71M Load
Range C Tubeless tire
violently exploded while the tire was being beaded/seated onto a tire rim
using a direct compressed air
(pneumatic) line.
Two (2) employees were struck by the tire and rim while performing the
tasks and sustained serious bodily
injuries to the hands, eyes and facials from the incident.
Among other methods, one feasible and acceptable means of abatement method
is to:
1. Use of appropriately designed, engineered and approved protective
barrier, restraining device and/or cages.
2. Use of safe and proper portable compressed bead seaters, such as (but
not limited to) the CH-5 Cheetah
bead seaters and the White Blaster.
3. Follow the manufacturer's recommended operating procedures, guidelines
and rules including not over-
pressurizing the units, having functional gauges and valves, safety
devices.
4. Train employees in safe and proper use of equipment.
5. Utilize other safe engineering practices.g
