Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards associated with crane deficiencies: (a) Shop Area; The #1 underslung hoist crane had the following deficiencies: 1. The wire mesh on the pendant control cable was broken away at the control box. 2. The wire rope was not inspected on a monthly basis with certification records. 3. The running rope did not seat properly in the lower sheaves (it ran off center). 4. There were two different load ratings for the one hoisting unit. 5. Monthly inspections of the hook, with certification records, were not conducted. 6. A preventative maintenance program based on the manufacturer's recommendations was not established.
