Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)Hazard of fall when working on pipe rack foundation that was not substantial enough to hold the amount of weight that would be placed on it during well workover operations at Well #28-3-1 owned by Coho Resources, Inc. on December 6, 1995. Among other methods one feasible and acceptable abatement method to correct this hazard is install a manufactured pipe racking mat or install racking foundation designed to withstand the maximum anticipated load.
