Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm in that
employees were exposed
to the hazard of exploding LP gas:
(a) In the Kitchen, three utility gas outlets were not securely closed
gaslight with a threaded plug
or cap.  Reference is made to paragraph 3.8.2, Cap All Outlets, of NFPA
54-1969, Standard
for the Installation of Gas Appliances and Gas Piping.  Gas utilization
equipment had been
disconnected from outlets that were not being reused and the outlets were
not closed gaslight.
The uncapped/unplugged outlets were located in the kitchen, on the south
wall of the
establishment, on or about 1/29/1997.
ABATEMENT NOTES:
Among other methods, one feasible and acceptable method to eliminate this
hazard is to follow
the American National Institute Safety Standard National Fuel Gas Code -
Safety Requirements
(ANSI Z 223.1 - 1996), Section 3.8.2, which states as follows:  "Each
outlet, including a valve
or cock outlet, shall be closed gaslight with a threaded plug or cap
immediately after installation
and shall be left closed until the gas utilization equipment is connected
thereto.  When equipment
is disconnected from an outlet and the outlet is not to be used again
immediately, it shall be
closed gaslight".
