General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the recognized hazards associated with falling objects. Specifically: a) Adequate inspections and maintenance of the underhung hoist and hoist roller chain were not done to prevent failure of the hoisting mechanisms and subsequent unexpectedly falling loads such as occurred on March 23, 1992 at the truck tank repair and resale facility that resulted in the death of an employee. Feasible methods of abatement include but may not be limited to the following: 1) Periodically inspect, maintain, and document inspection and maintenance of the hoisting mechanisms as per ANSI/ASTM B30.16- 1987 and latest manufacturer's guidelines. 2) Prevent twisting and subsequent failure of the hoist roller chain by checking to verify that all links flex easily (including the chain connection that should move freely in the slots of the lower block assembly) as stated in ANSI/ASTM B30.16-1987 and the latest manufacturer's guidelines. 3) Replace and splice roller type load chain only with roller chain that is specially designed for hoisting service as per the latest manufacturer's guidelines and not with chain designed for power transmission service.
