employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to crushing hazard:
a)  Mountour Road and Barfield, East Keating, PA
jobsite:   Caterpillar 583H Side Boom Pipe Layer Co.
No. 347, SN61A848, bidirectional equipment was not
equipped with a horn or a back-up alarm, October
29, 1991.
AMONG OTHER MEHTODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS .....
Install horn for forward travel and back-up alarm
for reverse travel.
