Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to crushing and fall hazards:
a)  Langley Vertical Lift Model EH-102:  gates and
side enclosures had been removed, exposing employees
to crushing hazards between the lift platform and
the pit wall, the platform and its landing pad in
the basement; as well as a fall hazard of 105 inches
from the first floor to the lift platform at rest
in the pit (basement level), November 6, 1992.
FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT
THESE HAZARDS ARE:
(1)  Replace the Logan double-hinged wire gate at
the first floor level and the double-hinged wire
gate and side enclosure at lower level if these
safeguards (original) including interlocks and
accident prevention signs meet ANSI Standard
A92-3-1980, Revision of ANSI 92.3-1973.
(2)  Conduct a current inspection of all electric,
hydraulic and mechanical component parts of the
lift.
(3)  Instruct all employees in operating the lift
and potential hazards when safeguards are removed
or not in operating condition.
