Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to a fall hazard:
a)  An employee was exposed to an 18 foot fall hazard
while shimmying across the "I" beam understructure
of the bridge along Route 80, June 22, 1992.
b)  An employee was exposed to a 15 foot fall hazard
while sitting on the outside of the vertical support
of the cantenary scaffold, thereby balancing the pic
while the far vertical support was being installed,
June 22, 1992.
c)  Exit 37 and Route 80 - employees performing grit
blasting from a cantenary scaffold located 15 feet
above the ground were exposed to a fall hazard in
that a safety belt and lanyard were not used, June
22, 1992.
Among other methods, some feasible and acceptable
abatement methods to correct this hazard are:
1)  the use of scaffolds to perform such work:
2)  the use of a safety belt and lanyard.
OR IN THE ALTERNATIVE:
29 CFR 1926.28(a):    Appropriate personal protective equipment was
not worn by employees in all operations where there was exposure to
hazardous conditions:
a)  An employee was exposed to an 18 foot fall hazard
while shimmying across the "I" beam understructure
of the bridge along Route 80, June 22, 1992.
b)  An employee was exposed to a 15 foot fall hazard
while sitting on the outside of the vertical support
of the cantenary scaffold, thereby balancing the pic
