Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to hazards of electrocution or amputation:
(a)M/V Clipperton:  Employees who perform maintenance on process equipment
under the companies lockout/tagout program are in danger of electrocution
or
amputation should they return to work on a piece of equipment from which
the
Chief Engineer has removed the lock or tag without notice.
Among other methods, one acceptable and feasible method of abatement would
be to amend the lockout/tagout program to ensure notification of employees
when
the Chief Engineer removes another employee's lock or tag.
(b)M/V Clipperton:  Three tagout tags on the electrical panel in the
boiler room
were used to remove equipment from service for extended periods of time
rather
than for protection of an employee working on the equipment.  This
violates the
companies written lockout/tagout program.
Among other methods, one acceptable and feasible method of abatement would
be to retrain employees in the lockout/tagout program to ensure that locks
and
tags are used only for employee protection.

(a)M/V Clipperton:   Employees using the outlet above the work bench to
power
tools were exposed to shock hazards due to the reversed polarity of the
outlet.
Among other methods, one acceptable and feasible method of abatement would
be to rewire the outlet with the proper polarity.
