Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to a recognized hazard of electrical shock
or burns:
(a)P/V Crusader:  The employer's lockout/tagout program did
not contain lockout/tagout procedures for each individual
pieces of machinery or equipment which had more than one
source of energy or the potential of stored energy.  This
condition exposed employees to possible electrical shock
or burns in the event all energy sources were not
isolated during machinery and equipment repair.
Among other methods, one feasible and acceptable
abatement method to correct this hazard would be to
develop, document, and utilize energy control procedures
for each individual machine or piece of equipment.
