Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a recognized hazard of electrical shock or burns: (a)M/V Atlas: Employees working on the processing deck had occasion to work near a Square D electrical panelboard (#2208) that was not dead fronted. Employees were exposed to possible electrical shock or burns in the event of accidental contact with 120/208 volt bare electrical conductors. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to replace the missing dead front. (b)M/V Atlas: Employees working on the processing deck had occasion to work near a 120 volt factory lighting panelboard that was not dead fronted. Employees were exposed to possible electrical shock or burns in the event of accidental contact with bare electrical conductors. Among other methods, one feasible and acceptable abatement method to correct this hazard is to replace the missing dead front. (c)M/V Atlas: Employees working on the processing deck had occasion to work near a 460 volt electrical distribution panel that was not dead fronted. Employees were exposed to possible electrical shock or burns in the event of accidental contact with bare electrical conductors. Among other methods, one feasible and acceptable abatement method to correct this hazard is to replace the missing dead front. (d)M/V Atlas: Employees were required to work on air compressors, refrigeration units, pumps, cranes, and other equipment without the benefit of a lockout/tagout program to control energy sources. This condition exposed employees to the inadvertent release of energy and possible injuries during maintenance operations. Among other methods, one feasible and acceptable abatement method to correct his hazard is to develop a lockout/tagout program using the requirements specified in 29 CFR 1910.147.
