inadvertent restart of equipment during maintenance procedures:
(a)   F/V Response:  In the processing area, mechanical, electrical
and engine rooms, there were no written procedures, rules or
training developed or implemented on energy controls for
employees' protection and training against hazards such as
bruises, contusion, fractures and death when they work on
repairing processing equipment, mechanical, electrical and
engine room equipment in accordance with 29 CFR 1910.147.
Among other methods, one feasible and acceptable method to
correct this hazard would be to develop and implement a
written
hazard training program on lockout/tagout.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to
electrical shock, burns and/or electrocution:
(a)   F/V Response:  In the engineer space, a breaker panel was
missing the door, exposing employees to contact with live
electrical parts.  The breaker panel contained breakers
labeled
"20 hp chiller" and "45 hp chiller."  This is a 220-volt
panel,
which had three blue lights installed at the top and all were
lit.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to replace the door.
(b)   F/V Response:  In the engineer space, a breaker panel was
missing its front, exposing employees to contact with live
electrical parts.  This cabinet also was missing "knockouts"
in
the side and bottom.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to replace the cover.
(c)   F/V Response:  In the engine room, a breaker panel with a
common
center bus bar was missing its cover, exposing employees to
live
electrical parts.  The 220-volt panel was located adjacent to
a transformer across from the diesel generator.  This panel
contained the breakers for the crane and bait freezer.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to replace the cover.
(d)   F/V Response:  In the engine space, a breaker panel attached
to
storage bins was missing its cover, exposing employees to live
electrical parts.

(e)   F/V Response:  In the engine room, an open panel box was
located
along the wall adjacent to the hot water heater.  The 220-volt
panel was divided into three sections, and a conduit entering
the panel from the bottom was broken.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to replace the cover.
(f)   F/V Response:  In the engine room, a 120-volt light switch,
adjacent to the hot water heater, was missing the cover plate,
exposing employees to live electrical wires.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is replace the cover.
(g)   F/V Response:  In the engine room, an electrical "J" box,
located on the ceiling adjacent to the water line, was missing
the cover material in proper knockout covers and lacked
conduit
and wire strain relief devices, resulting in employees
exposure
to live electrical parts.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to replace the cover, provide
proper knockout cover and conduit.
(h)   F/V Response:  In the engine room, a 220-volt electrical
meter,
painted blue, was missing the cover to the "J" box, exposing
employees to live electrical parts.

method to correct this hazard is to replace the cover.
(i)   F/V Response:  In the engine room, a 120-volt junction box was
missing its cover.  The junction box was located adjacent to
the
compressed air expansion track beneath a horizontal stretched
member used for storage.  Wired into this box was a light
switch
on a flex cable about 18 inches long.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to replace the cover.
(j)   F/V Response:  In the engine room, exposed 120-volt wires with
wire nuts to overhead fan unit.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to put wires in the conduit
and
"J" box.
(k)   F/V Response:  In the engine room, an open "J" box, dangling
at
the end of flexible conduit, was missing a cover and had wires
pulled out, exposing employees to contact with live electrical
parts.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to replace the cover.
(l)   F/V Response:  In the freezer space, a 120-volt incandescent
light was hanging loose from its box, exposing employees to
live
electrical parts.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is reassemble the light fixture.
