Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a recognized hazard of electrical shock and burns:
(a)F/V Yukon Princess:  Employees working or walking on the starboard side
of the
vessel were exposed to the live electrical conductors that were pulled
from an
electrical duplex box strain relief and the ground wire was severed at the
break.
Among other methods, one feasible and acceptable method of abatement would
be to repair the broken strain relief and correct the ground wire.

(a)F/V Yukon Princess:  Employees working in the vessel tool/compressor
room
were exposed to live 240 volt electrical conductors on the State 510E hot
water
heater where the protective cover was not in place to prevent employee
contact
with live electrical conductors.
Among other methods, one feasible and acceptable method of abatement would
be to reinstall the cover over the controls and live conductors.

inadvertent restart of
machinery:
(a)F/V Yukon Princess:  Employees working on equipment on the vessel such
as
compressors, conveyors, electrical control, etc. were exposed and
subjected to
possible personal injury because the employer failed to provide a
lockout/tagout
energy control program to protect employees.
Among other methods, one feasible and acceptable method of abatement would
be to implement a lockout/tagout program for the vessel.
