Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to falling warps:
Mill #5 Weave room #3 - Chain Hoists for warps - Spreader bars are
not secured by shackles, nuts or bolts with cotter pins.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to replace all nuts, shackles or bolts,
train all affected employees in properly securing the spreader bar
after each warp change and routinely inspect the equipment to ensure
that some type of locking device is being used and that established
procedures are being followed.
