Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion and fire due to inadequate equipment, training and procedures: (a) #3 Blow tank/accumulator - Prior to cutting and welding employees were not provided with equipment, training, or procedures on what steps to take to insure that the vessel was completely free of flammable vapors. On or about February 3, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide training and instruct employees on atmoshperic testing equipment and establish written testing procedures that employees must followed before cutting and welding on tanks that contained flammable vapors.
