Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: (a) Jobsite, a 1 ft. X 24 ft. scaffold board; was used as an employee runway to bridge the gap between an elevated truck bed and the edge of a roof located 17 ft. above the ground and was not secured from movement at the roof edge nor at the truck bed. The scaffold board was being used by employees to load materials such as 4 ft. X 8 ft. sheets of plywood between the roof and truck. One or more feasible and acceptable abatement methods to correct this hazard is to; (1) Secure the scaffold board to the truck bed and the roof. (2) Ensure the keys to the truck door and ignition are in a lock box with access controlled by the person(s) using the lift bed function of the truck. (3) Ensure the truck is secured from movement with bidirectional blocking of at least two sets of wheels. (4) Ensure lifting bed controls are key controlled and those keys are under supervisor control when employees are working in the elevated bed of the truck. (5) Ensure the lifting mechanism is prevented from premature release due to a failure in any singular mechanism.
