Executive Order 12196 Section 1-201(a) of the Occupational Safety and Health Programs for Federal Employees: The Head of the Agency did not furnish employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees operating the cranes, were exposed to crushing injuries from falling loads or falling crane members off of two (2) overhead underhung cranes in DOL/Bldg 8000; Cranes in which a complete periodic inspection of, had not been conducted since September 1989. Among other methods one feasible and acceptable method to correct the hazard is to have complete periodic inspections of the cranes conducted in accordance with ANSI B30.11b - 1990; Monorails and Underhung Cranes; Chapter 11-2 titled Inspection, Testing and Maintenance.

Executive Order 12196 Section 1-201(a) of the Occupational Safety and Health Programs for Federal Employees: The Head of the Agency did not furnish employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that the area that is in front of the dock area and serves as a walkway into the main work area as well as the pathway between the breakroom and main office, has an uneven walking surface exposing employees to tripping and falling hazards. Among other methods, one feasible and acceptable method to correct the hazard is to even-out the floor area, making the surface smooth with no bulges or bumps.
