Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish his employees employment and a place of
employment which were free from recognized hazards that were causing or
likely to cause death or serious physical harm to employees in that
employees were exposed to:
a) Equipment failure in that the eliminator personnel lift
was not being maintained in accordance with the manufactur-
ers recommendations in the safety operation, service and
parts manual for the ML4-24HD eliminator, serial #24285. The
equipment had the following defects:
1. The hydraulic leaks located on the stablizing
outriggers were not repaired.
2. The electric control box located on lift platform had
weakened wire connections.
Among other methods one feasible and acceptable method to correct the
hazard is to establish a maintenance program for the lift which would
comply with the safety, operation, service and parts manual for the
ML4-24HD eliminator.
