Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a) At the time of the inspection, employees were using the manually
operated manlift to access
the upper levels of the elevator. The following deficiencies were noted;
1. The manlift was not periodically maintained and inspected by qualified
personnel.
2. The driving machine-brake was not tested with both empty car and rated
load in the car.
Failure of the elevator would create a fall from elevation.
Among other methods, one feasible and acceptable method to abate this
hazard is to comply with
ASME A17.3 Section 1.5(a)&(b). That standard references the following
sections of ASME
A17.1 Sections 8.6, 8.7, 8.10, & 8.11.
Abatement certification and abatement documentation is required for this
violation.  The
documentation should include written verification of abatement, applicable
measurements or
monitoring results, and photographs or videos which you believe will be
helpful.  The abatement
certification sheet is enclosed with the citation.
