Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to hazards of being crushed by the concrete and concrete form
work as a result of form work being loaded beyond its safe working
capacity:
(a)  Digital Building Construction Site, Hudson, MA:  On or about
1/7/93, adjustable screw support shores along the west edge of
the concrete form work for Fab 6.1 waffle slab were loaded beyond
the safe working load.
(b)  Digital Building Construction Site, Hudson, MA:  On or about
1/7/93, the wood support blocks for the column bases along the
west edge of the concrete form work for Fab 6.1 waffle slab were
loaded beyond the safe working load.
(c)  Digital Building Construction Site, Hudson, MA:  Erection
of the concrete form work used on site was performed without
adequate documentation from either the manufacturer of a person
competent in the field of concrete form work design for items
such as but not limited to the form work base supports, column
flange fasteners and girder splices.
NOTE:  Among other methods, one feasible and acceptable abatement
method to correct this hazard is to have either the manufacturer
of the concrete form work or a competent person in the design
of concrete forms develop accurate and complete instruction and
specifications for the total concrete form work to be used.
