Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Job site: On 05/24/94, employees were potentially exposed to the hazard of being struck by a concrete wall panel as the panel was being lifted into place by a crane. The hazard resulted from the fact that the lifting inserts had not been installed so that they had sufficient strength to support the panel. One feasible and acceptable method of correcting the hazard is to insure all lifting inserts are installed in accordance with the manufacturers specifications.
