Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a) On or about 8/20/93, at Minto construction work site, Boyton Beach, FL, employee forming at the tie beam wall was exposed to a 22 foot fall hazard. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: (a) Employees to utilize ladders secured to tie beam wall. (b) Scaffolds shall be erected while forming and pouring tie beam wall under 25 feet. (c) Out rigger platform to be erected on the working floor. (d) Interior scaffold (manually propelled mobil). (e) Pump jack scaffolds to be utilized where feasible. (f) Provide safety belts, lanyards and a secure place to tie
