The alleged violations below (9a - 9b) have been  grouped  because
they  involve similar or related hazards that may increase the
potential  for  injury resulting from an accident.
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to the
hazard of explosion and or fire:
(a)    On or about August 12, 1992, there was an explosion at the
Alloy and Casting area as a   result of mixing Aluminum and
Sodium Nitrate out of sequence.
Among other methods, on feasible and acceptable abatement method to
correct
this hazard is to develop a written standard operating procedure manual
that
details the procedures to be used in mixing the chemicals.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to the
hazard of falling onto running conveyors and being crushed by equipment
that
the conveyors lead to, on or about August 12, 1992.  Remotely and
automatically controlled conveyors where operator's stations were not
manned
or were beyond voice and visual contact from drive areas, loading areas,
transfer points, and other potentially hazardous locations on the conveyor
part were not furnished with emergency stop buttons, pull cords, limit
switches, or similar emergency stop controllers, all easily identifiable,
in the immediate vicinity of such locations:
(a)   Saw Mill area - The 300 feet long conveyor was not provided
with
emergency stop or other acceptable device.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to install an emergency stop alongside the conveyor, as
required by ANSI B20.1 - 1972, Section 5, 5.15.2 Requirements, 5.15.2.5.3.

hazards, on or about August 12, 1992:
(a)   Employees repairing the roof over the Alloy and Casting Area
working approximately 25 feet from the ground were not
provided
with fall protection.
Among other methods, one feasible and acceptable method to correct this
hazard is to provide catch platforms and/or fall protection such as safety
belts and lanyards.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to the
hazards of fire and/or explosion.  Positive identification of the contents
of the piping system were not by lettered legend giving the name of the
contents in full or abbreviated form, and arrows were not used to indicate
the direction of flow where pipes pass through walls or floors and at
intervals on straight pipe runs sufficient for identification:
(a)   Overhead pipes containing natural gas, oxygen and
water located in the Blasting, Casting and Alloy
areas were not labeled to indicate the contents and
direction of flow, on or about August 26, 1992.
One feasible and acceptable abatement method is to label and color-code
all
overhead piping systems to identify the contents and direction of flow as
required by ANSI A13.1-1981, Section 3.1, Scheme for the Identification of
Piping Systems.
