Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of falling from heights ranging from approximately 10 to 40
ft., on or about May 1992:
(a) The employee(s) operated the Elliot Hi-reach Bucket truck,
while its hydraulic hoses were leaking.  The employee(s)
where inside the bucket, working at heights approximately
40 ft.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to repair and/or replace any leaking hydraulic hoses
prior to employees operating the lift truck.

(b) The Calavar Self-Propelled Condor 66, Model# 66Q8 was pulled
by a front end loader via wire rope, up ramps with slopes of
greater than 14 degrees, and while an employee was inside the
Condor's bucket, approximately 10 ft. in the air.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to slope the ramps to no more than 14 degrees, as
stated in the manufacturer's operating manual.
