General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the concrete at the ground level: a) Northeast roof over the second floor of the main building, south of the mental health building - an employee was painting an exhaust duct at the roof edge while standing on the parapet edge, 33 feet above the adjacent roof and 45 feet above the ground without utilizing fall protection. Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide and use a safety belt/harness and lanyard secured to the structure.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of impalement on the lightning rods at the roof edge attached to the parapet: a) Northeast roof over the second floor of the main building, south of the metal health building - an employee was painting an exhaust duct at the roof edge while standing on the parapet edge. A lightning rod 18 inches high was attached vertically to the inside of the roof parapet within 3 feet of the employee, exposing him to an impalement hazard. Among other methods, one feasible and acceptable abatement method to correct this hazard is to cover the lightning rod with wood or plastic pipe slipped over the rod.
