General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by vehicles which have an obstructed view to the rear and no reverse signal alarm: a) The three Commercial Carrier Corporation Concrete mixer trucks which operated in the yard between the ready mix plant and the form pouring area. b) The Michigan front-end loader which operated in and about the ready mix plant. Among other methods, feasible and acceptable methods to correct this hazard would be to: 1) Install a reverse signal alarm; or 2) Provide a signal person or observer to direct the concrete mixer truck or front-end loader operator when it is safe to back-up.
