Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing or amputation hazard on a poly machine due to inadequate training procedures: Poly Machine Dept. - On or about 01/07/92, a trainee, left alone to operate the machine, had his right index finger accidentally amputated by poly machine #5. One feasible method of abatement is to have the company's safety and health committee (comprised of labor and management) and the exper- ienced operators sit down together to develop an effective training program for these operators.
