Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to corrosive contact hazards from possible leaks, ruptures,  or
spray-outs from valves,  pipelines, vessels,  and  equipment   handling
corrosive materials.  The following  conditions  contributed   to   the
existence of these hazards:
Chlorine Dioxide Process Area (Cape, basement) - Joints in
sulfuric acid pipelines routed above doors, walkways, and
other protruding occupied areas, did not have either covers
to protect employees from accidental sprayouts, or were not
routed away from occupied areas.
Note:  Lines are routed over personnel door.
