employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
explosion hazards:
FILLING ROOM: AN OIL LUBRICATED VACUUM PUMP WA IN THE MANIFOLD PIPING
SYSTEM USED TO FILL OXYGEN CYLINDERS. OIL WAS FOUND IN THE PIPING
BETWEEN THE FILLING MANIFOLD AND THE VACUUM PUMP PRESENTING AN
EXPLOSION HAZARD.
1) INSTALL AN OILLESS VACUUM PUMP OR INSTALL ONE THAT USES A LUBRICANT
COMPATIBLE WITH OXYGEN, I.E., A LUBRICANT THAT CANNOT PRODUCE AN
EXPLOSIVE MIXTURE WHEN COMBINED WITH OXYGEN.
2) DEVELOP AND IMPLEMENT AN EFFECTIVE PREVENTIVE MAINTENANCE PROGRAM
FOR ALL PUMPS, EQUIPMENT AND PIPING USED IN THE OXYGEN FILLING
SYSTEM TO ENSURE THEY ARE IN PROPER OPERATING CONDITION AND TO
ENSURE THERE IS NO CONTAMINATION IN THE SYSTEM.
3) EMPTY AND CLEAN ALL COMPRESSED GAS CYLINDERS OF MEDICAL GRADE OXYGEN
AND COMMERCIAL GRADE OXYGEN THAT WERE FILLED USING THE MANIFOLD
SYSTEM IN THE PLANT AND MAY HAVE BECOME CONTAMINATED WITH OIL.
COMPRESSED GAS ASSOCIATION PAMPHLET 6-4.1 "CLEANING EQUIPMENT FOR
OXYGEN SERVICE" PROVIDES GUIDELINES ON PROPER CLEANING PROCEDURES.
