Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Willow Avenue & East 133rd Street - On 10/7/89 - Employees were exposed to fire and explosion from a 13,000 volt energized feeder cable. The feeder cable 3M60 was classified as a type II "D" Fault and was located in manhole #11841. The feeder cable was not immediately removed from service during the blanketing operation. One feasible and acceptable method to correct this hazard is to remove "D" Fault from service and do not enter manhole without deenergizing the feeder cable.
