Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) Basement Level - On 3/9/90 - Employees were exposed to being crushed
by Atlas Dumbwaiter FDO-500 installed at location. Dumbwaiter was
permitted to operate with its hoistway door open (basement level)
without the operation of electrical and mechanical interlock and
contacts.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to return all mechanical (keeper) interlocks to
doors, re-install electrical contacts and place wiring back into the
safety system for the basement level. Insure that the dumbwaiter car
will only operate when both car and hoistway doors are in the closed
position.
