Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to FALL HAZARDS: a) East side of Carrier Dome floor, on or about 7/30/02: All employees were not trained in the safe and proper tower erection procedures. ABATEMENT NOTES: Ensure that all employees are adequately trained in the proper and safe use of tower equipment per James Thomas Engineering 2 Ton Ground Support Tower-Operating Instructions, Page 1. b)East side of Carrier Dome Floor, on or about 7/30/02: An employee was permitted to climb a tower prior to having all truss bolts and nuts installed and secured(tightened) to prevent the tower from tipping over. ABATEMENT NOTES: Ensure that all truss bolts and nuts are secured prior to an employee climbing the tower per James Thomas Engineering 2 Ton Ground Support Tower-Operating Instructions Page 5, paragraph 10 and paragraph 11. c)East side of Carrier Dome Floor, on or about 7/30/02: An employee was permitted to climb a tower to perform work without having a proper safety harness. ABATEMENT NOTES: Ensure that any employee permitted to climb towers is wearing a proper safety harness(body harness, lanyard, shock absorber) and uses this harness when performing work on towers per James Thomas Engineering 2 Ton Ground Support Tower- Operating Instructions Page 5, paragraph 11. d)East side of Carrier Dome, on or about 7/30/02: An employee was permitted to climb a tower that had not been assembled, joined together, and erected by competent persons. ABATEMENT NOTES: Ensure that all persons involved with the assembly, joining together, and erection of trusses and towers are fully competent, meaning that they are fully capable of recognizing hazardous conditions and have the authority to take prompt corrective measures to eliminate these hazards per American National Standards Institute E1.2-2000, Entertainment Technology: Design, Manufacture, and Use of Aluminum Trusses and Towers, Page 9, Paragraph 5.5.3.
