the employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to being struck by or crushed by an
unsecured load:
On or about 03/10/95 employees in sub assembly, were installing an
approximately 3500 pound prefabricated steel rail car side onto a
cement hopper rail car, Serial # 732196. The load (rail car side)
attached to the overhead hoist was released without it being
adequately and properly secured.
One feasible and acceptable means of abatement is to connect the
fit up chains across the top and of the rail sides before removing
the crane, or before employees are allowed to perform work in the
area of the unsecured rail car side.
