Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
remotely and automatically controlled conveyors with potentially
hazardous locations on the conveyor path:
a) Employees were observed vacuuming coke debris in the coke breeze
belt conveyor pit (at the United States Steel Division of USX,
Fairless Works)  in the blast furnace department adjacent to ingoing
nip points created by the conveyor belt and rollers and emergency
stop devices were not in the immediate vicinity of the hazards, June
19, 1989.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to install and emergency stop device in the
immediate vicinity of the hazardous locations so that the device acts
directly on the control of the conveyor.  The emergency stop device
shall depend on the stopping of any other equipment and the device shall
be installed so that it cannot be overridden from other locations.
