Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
a fall hazard:
a) Rolling Division, 80" MIll, Coil Pit Area - Employee working on top
of rolls was not tied of or secured from falling 20 feet to floor
below, August 23, 1989.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to utilize an employee fall protection system
in accordance with 29 CFR 1910,104, entitled "Safety Belts, Lifelines,
and Lanyards".

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
improper rigging practices:
a) Employees were observed working from a swing scaffold at the blast
furnace tressel with rigging that had several problems, thus exposing
employees to an approximate 40 foot fall hazard. These poor rigging
practices included the scaffold's wire rope suspension system was not
protected from damage by sharp edges, the wire rope eyes did not have
thimbles, the shackles were installed in such a manner that the pins
could roll under the load and unscrew causing the scaffold to fall
and the shackles were exposed to lateral forces as the load (wire
rope) was not centralized on the shackle pins, June26, 1989.
Among other methods one feasible and acceptable abatement to correct
this hazard is to install thimbles on wire ropes, protect wire ropes
from physical damage with softener, secure shackle pins to shackle with
wire or use a cotter pin type shackle and pack the pin with washers to
centralize the shackle.
