Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
health hazards and hazards of physical injury resulting from the release
of chlorine gas into employee work areas.  The following condition
contribute to the existence of this hazard:
a) The ventilation system of the clorination room was not designed to
provide an adequate amount to makeup air, 4/6/90.
b) There was no means of viewing the chlorination system from the room
exterior, 4/6/90.
c) The entrance door was not provided with panic hardware, 4/6/90.
d) There were inadequate means of detecting and warning employees of
chlorine gas leaks, 4/6/90.
Feasible and acceptable abatement methods among others include:
1) Provide for air inlets through louvers near the ceiling such that the
temperature of the makeup air does not adversely affect chlorination.
2) A clear, gas tight, shatter resistant inspection window should be
installed in an interior wall or exterior door to permit the
chlorination system to be viewed without entering the room.
3) The door should be provided with panic hardware to permit a ready
means of exit to the building exterior.
4) Installation of a chlorine leak detection monitoring and audible
alarm system.
