Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did
not furnish employment and a place of employment which were free from
recognized
hazards that were causing or likely to cause death or serious physical
harm to employees
in that employees were exposed to the hazards of explosive release of hot
caustic material
and to flying debris:
a.At the establishment, the owner or user did not maintain a record of all
repairs made to Digesters 1-8.  Among other methods, one feasible and
acceptable abatement method to correct this hazard is to implement a
recordkeeping program in accordance with the provisions outlined in
National Board Inspection Code (NBIC-ANSI/NB-23), Chapter III,
Paragraph R-402.2, which requires records be maintained of all repairs to
digesters.
b.At the establishment, repairs to pressure vessels were initiated without
the
authorization of the Inspector on Digesters 1-8.  Among other methods,
one feasible and acceptable abatement method to correct this hazard is to
follow the procedures in National Board Inspection Code
(NBICANSI/NB-23),
Chapter III, Paragraph R-301.1, which require repairs to
boilers or pressure vessels be initiated by the authorized Inspector.
