Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. The hazard of being struck by the load. CSX facility compound; Employee standing inside steel cased, circular shaft (depth = 14' diameter =15') was seriously injured when a 48" cast iron, bell section of pipe, being lifted from line position with a 3/4" wire rope single choker hitch secured to the bucket of a Linkbelt backhoe, swung out of control crushing against the steel casing, on or about June 5, 1991. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use one or more guide ropes or tag lines to prevent rotation or uncontrolled motion of the load being lifted and make sure employees stand clear which the load is being lifted.
