Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that emloyees were exposed to:
a) In all plant area - shop made lifting hooks, spreader bars, and
brackets are being used in lifting loads and have not been load
tested and rated. Chains were welded to shop-made hooks in at least
instance.
One feasible method of abatement would be load testing and rating of
spreader bars and brackets.  Removal from service of shop made hooks
to which chain links were welded.  Maintain documentation showing load
rating methods and decision.
