General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by material: a.On or about August 28, 1997, Gantry Crane/Mold area, the metal piping supplying 135-175 psi of compressed air to the batch plant mixer, concrete molds, and concrete forms; had leaks in the piping, primarily at metal piping couplings. If the pipe or coupling ruptured; airborne metal, rocks, or other debris may result in a serious employee injury. Feasible and useful abatement methods for reducing this hazard, among others, are: 1.Implementation of preventative maintenance program with emphasis on inspection and replacement of existing compressed air metal piping. 2.Implementation of training procedures such that employees are made aware of compressed air hazards, informed of proper personal protective equipment to be worn when exposed to compressed air (if necessary), perform required lockout/tagout procedures when working on piping, and are encouraged to notify management of unsafe conditions. 3.Ensure that industrial truck and other equipment operators do not drive over compressed air piping unless ramps are installed over the metal piping. ABATEMENT VERIFICATION IS REQUIREDTION
